What’s working nowadays with SEO?

As we all know, what was true a couple of years ago about SEO, it’s not true now.

What is in your opinion the best way to achieve better standings with search engines (in the following topics):

How many keywords?
Keyword type…, small caps, caps? Comma spacing, “ABC, ABC” or “ABC,ABC”?
Word density?
Page title?
File name?
Site description?
ALT tags?

@Steve_SmithNov 28.2012 — Search Engine Optimization is highly specialized process of building a successful website. We say successful because if a commercial website cannot be found in the major search engines, it is not successful, it just isn't doing it's job.
