Form with message and redirect to thankyou.html

I have finally found a working PHP form online to submit a file with the form that delivers the file attachments to my inbox as an attachment. That part is perfect! Now, I am trying to go a step ahead and do two things: 1.) include the label for each additional text field I add in so that I know what each users’s typed in entry is for and 2.) to also redirect the form to the thankyou.html if everything is successful or to a tryagain.html so the user can click on a link to go back to the form and start over. Can someone please help?

Here is the specfic code for the first issue – how do I include the label names and also start a new line for each one. I tried using the <p></p> but it didnt work:
