Does anyone know of a Open source or Even a pay Script That Will Do This?

OK i have a client that needs a email script, he owns a kiddie park and wants people to be able to go to the website and sign up for birthday events where as he will email them something on a upcoming birthday.

the script must have a data base that retains the emails, have the ability to mass email , so in essence i guess a opt in email script, that can be edited to require name, dob, address, phone ect…
