countries and states and zips

this is not as much a php question as a internet/ web design question. I am building the form with PHP so here we go…

I was wondering if there is a DB I can download that has all the states and providences and also all the countries that most forms on the net have in them?

Also, I was wondering the same for ZIP CODES… Also with Zip Codes… how can you write something so that you can have a radius search…such as…

everybody with a zipcode within 10 miles of 08080 or something for example?
