Activation Required in Email for Activation

hello,
i m a newbie in PHP
can u help me plz.

i am having a page which shows a form with some fields like:-
1) Name
2) Address
3) Email ID
…
….

On Submitting that form a mail is sent to the designated given Email-ID.
What i want is to have a link in that mail such that when that user clicks on that link then and only then his account should be activated to prevent spam..

can u give me some information/code regarding that
