PHP calendar with form insert

hi, Im looking and have searched for this, a code or site where i can get the code to do much like the date function on PHPmyAdmin. Where as you hover over a calander image/link which brings up a small little calander then the date the you click on will populate a certion specified text input?

Has anyone got or seen a working version of this?
