Good looking form result

I’d like to create a form that, when submitted, emails a form page that looks good. I was looking at [url]http://www.planetpdf.com/developer/article.asp?contentid=6492[/url] That’s from 1999, though and I’m not sure if that would end up giving me a blank PDF document or not.

Anyone have any experience to this? I’d just have the server create a PDF document then print it, but the server isn’t in the office location and the file does need to be emailed after it’s been created. I’d like it to just print out looking nice so that a person wouldn’t have to spend time redoing the document.
