checklist form, post to html?

Hello, I have a form written in dreamweaver. It lists all the courses required to complete a two year degree in a specified major.

I want High School students to be able to access the form, check the boxes in front of the courses they’d like to take through distance learning at their local high school and then click submit.

After “submit” I want a page to display (nicely formatted, a table preferably) two segments. Segment 1 would be a list of the courses the student plans to take in HS. Segement 2 would list all the courses they’d have left to take at the college level.

Each row includes a course title with link to course description, a course catalog number and number of credits for the course. I’d like this to carry through to the results page. I don’t need it to email anyone or get stored in a database – just a nice clean print out for a kid to show his/her counselor, parent, teacher.

Any ideas on how this could work? Thanks so much. Julie

@julietauthorNov 23.2005 — Sheldon, you mention a database, I'm assuming MySQL. What part will the database play? Will the rows of data (course name, link, number of credits and course catalog number) have to be pulled from the database?

I was hoping I could just create an html form listing those rows and have it copy selected (checkmarked) data to another html page using a script -- totally bypassing the MySQL part of it. Not possible?

@SheldonNov 23.2005 — Oh yes it will be posiable, I just thought it would be easier with one. But not needed.

Do you want to post yor form from dreamweaver in [ php ] tags and we can try help you out some more with processing the code. How are you going to save their entries? Or are the students going to print it off?

@julietauthorNov 23.2005 — I've been looking for the form and can't locate it, so I'm attaching a file of my original layout scheme. Its a pdf that shows what my expectation is.

The form itself looks almost identical to this.

I do NOT need to save the data or autorespond email it to anyone. Just need a clean page for printing with the two segments as seen on the pdf.
