Data from form to a CSV file

Hello everyone,

I am a newbie, please bear with me…

I need to export data from a form and write them as a comma-delimited (.CSV) file so that whatever data was sent can be opened in Excel. The form would have fields like First Name, Last Name, Street, City and State.
