how to format a form

I have a form that i have and it basically looks like this

What is your first name[U]TEXTBOX[/U]
What is your last name[U]TEXTBOX[/U]
etc…….

What I want to do is have it so it looks like this….

What is your first name [U]TEXTBOX[/U]
What is your last name [U]TEXTBOX[/U]
What is your middle name [U]TEXTBOX[/U]
etc…….

how do i get that spacing correct?

that script gives them all the same spacing... as in

what i mean is that I want to align all the text boxes to the right side of the page, but align by the left side of the text box, but not all the way against the border.. and i want to align all the questions to the left, but not all the way against the border....
