Form disappears on window change

Hey, I’ve got a really goofy bug that I’m really hoping someone else has seen before.

What’s happening is that the script works just fine in the first time its opened in a window, but if you open it in a new window or (depending on browser), close the window and come back, the page cuts out the huge form in the middle.

I can’t for the life of me figure out why it’s doing that, have no clue what it means at all, and am really worried I broke something good. It never used to do that.

I did notice that the script that draws the form was still being called okay.

Any general ideas?
