Stopping input on a form if a checkbox is checked

I’ve got a form set up to collect some user data. There is a checkbox at the start that says “if your address details are the same as previous form then click here”. Then there are a number if input fields. If they check the checkbox I would like the other fields to become grayed out and inaccessible.

How do I do this??
