Reset form on Submit and Close Window

I want to put a CLOSE WINDOW button on the PHP form after you click on SUBMIT. I tried with Java Script, but couldn’t do it.

I also want when people click on the SUBMIT button the form to RESET after it submits the info. Thanks for the help.
