Looked over all groups could not find this, Very, very new at customizing my look and feel.

Where do I find a form (script, app, whatever) that will
[LIST]
show input fields:
name
email address
Has:
[LIST][*]
a SUBMIT button that records the information in the GOOGLE DRIVE spreadsheet,
says THANKS FOR SUBMITTING
redirects user (in what ever specified number of seconds after SUBMITing is clicked, or another GO TO Button) to the GOOGLE SITES page they have selected.

would like to know how to interface with or add to GOOGLE SITES. (what will this be Javascript – INSERT APP, then paste code??)
the rest like changing the background color, position the INPUT area customizing look and feel etc, I can figure out.
