How to create an inpage donation form?

Hello every one I am working for a political campaign and they want an in-page donation form like the follow 2 examples:

I’m pretty sure there is like a widget or something that makes this less painfull then hand-writing a donation front end that works. But I have NO idea where to start and by that I mean I’m not even sure what I should be looking up in Google to get some ideas about who offers these kind of customized donation front end you can just plug into a page.

@regansFeb 27.2012 — You can also use 123ContactForm web form builder, it enables you to quickly put up your donation form and disseminate it however it's convenient to you. For receiving payments, you can integrate your form with PayPal, Google Checkout or Authorize.Net (in the wizard, Settings > Payments).

They have an elementary template of donation form here, you can start your customization with this and add all the fields you need.

I have used it and recommend.
