Javascript in PDF for Auto Fill Date

I am in need of assistance. I have a PDF that is used as a timesheet that employees fill out. I have Period Start and Period End Date fields at the top of the PDF. When somebody enters in the Period Start date, I need the PDF to automatically fill in the rest of the dates for the two weeks, and am having no luck figuring this out on my own. Could somebody assist me with this?
