Contact form to PDF

Good afternoon and thank you in advance for looking at this. I am new to this forum and fairly new to PHP. We, the company I work for, have a good set of contact forms that are processed using PHPMailer-FE. Here is my question: Is there a software, paid or open source, that will take the responses of the contact form, and plug them into a pre-made PDF template, and then send the PDF in the email to the recipient? The contact form works great, but then once it is received, we have to transfer that information over to our PDF form. Just trying to simplify things here and cut back on tedious tasks.
