selection list not working

hi,

i have created a form. The selection list is not working i have attached my code and the screen shot for review. Hope someone can help me on this.

@KorMar 29.2011 — The [B]name[/B] must be borne by the SELECT element, not by the OPTION elements. After all you send a single pair of name:value to the server-side - the name of the select element, and the value of the selected option.
