Poll Question (3 forum topic)

ok i want to run a poll on my site. i want it to be in the header of the site and have it like any other poll you find on the web. a question, answers, and results of previous votes when you vote. i want it to be a forever running poll cause thats kinda my sites gimmick. my question is how do i do this.

i am running this question in HTML, CSS, and Javascript as i am unsure how to accomplish this

well i have 25 mysql servers, how do i go about setting 1 up to accomplish this?
