Wanting to make a Feedback Forum

Hi,

I am designing a website for a beauty company and they wanted a feedback page where people can freely express how happy or upset they are of the service with an option for it to be approved first.

I would like some sort of protection from spam as well, but not too sure what software is out to implement this.

I am looking on the internet on how to do this, but i keep finding feedback forms (contact forms) as apposed to what I want.

Can anyone suggest to me a good place to suggest to learn how to build it or any software that does this?
