Form class and mysql

Hello, this is my first post and I desperately need some help. I am using a php from builder class and I can’t seem to figure out how to fill mysql data into it.

I am trying to make a select dropdown list w/ mysql information below is the code I have to use. If some one could help me out with this it would be greatly appreciated. I am totally banging my head of the wall with this one. Thanks in advance.

$form->addSelect(“Select Box:”, “field12”, “”, array([COLOR=”Orange”]“option0” => “Option 0”, “option1” => “Option 1”, “option2” => “Option 2”[/COLOR]));

I need to get my mysql data into the highlighted area some how.
