PHP help

I am presently designing a website using php for my server side scripting and MySql for the database. I have designed the home page and I’m trying to create new registration but I keep having errors. I have checked the php code but can’t seem to get the error. kindly help

By elaborate I mean post error messages as well as your php and html code (database structure too, if necessary).
