Order Confirm Form on Javascriptsource.com

Hi,
I saw this great script on javascriptsource.com for order form processing, but I need to be able to alter it, add form elements like radio buttons, drop down menus and text area boxes! Can someone tell me how to do that or do you know of a script that is similar that I won’t have to alter as much? I’d really appreciate some help with this a.s.a.p. Here’s the link to the script!
