How to add comment to my link?

I have sample for image as link.
How to do in correct way image that shows as comment like in Excel…when user goes over link (not clicked).
I enclose my comments field.

2. How to add at the top of border cross like Description [B][SIZE="5"]X[/SIZE][/B] to close this image?

3. How to add [B]my div name [/B]as you included in js file?
