PHP upload and email form

Thank you for looking at this . I want to create a form with following functionality

  • 1.

    A select field for multiple recipients ( Recipient 1=[email protected] ; Recipient2=[email protected], etc)

  • 2.

    A textfield for comments

  • 3.

    An upload field for document

  • 4.

    Send (the document and comment get sent to selected recipients)

    • Can someone help me with

    A. The imput form
    B. the php processing script.
