I need quick help with this form code

Hello, I am a non-javascript web builder. I am working on a site which was previously built and has a javascript jumpbox form which works great. I have been able to change data successfully. The problem is that I need to have 2 forms (two drop-down menu jumpboxes) on the same page – jumping to a different set of pages. I can’t get the second jump box to work. I have a deadline in a couple of days.
