javascript check box working with next page to provide results

Hi everyone, i’m new to this forum. I’m a web designer more than a coder and am hoping a few of the ‘masters’ can help me out.

I am coding a page that has checkbox options. When the user checks the checkboxes (options they want such as downloading white papers), then put their information in and click submit.. the end results are displayed as links on the next page. I would imagine this would be a simple javascript code. I did find one that you may look at, except the result is e-mailed to the user i believe:

Any help much appreciated ?
