SharePoint Designer 2007 – form creation help needed

I’m creating my first form in SharePoint Designer 2007, because I have to… and want the submit button to fuction correctly.

All the tags show me that everything is correct.
Input, type, name and the mailto option includes not spaces, but because I don’t see the code, which drives me nuts, I also can’t be sure the info is going into my form.

Any ideas on what to check and what to do?
