Need to preview an order form before submitting

We are manufacturing customizable items for customers (i.e. they can add a name, date, picture etc). I am looking for some sort of script for an order form that will let people submit the custom information in their order, and preview it exactly how it will look on their item before they actually submit the order. I know it can be done as I have seen it on several sites, but most of them are linked to some other sort of forms submission site and I want it all from our own site. Can anyone help me? It’s been a long time since I have done any web development so I feel like I am starting out new again!
