Making JavaScript Order Forms

Can anyone help point me in the right direction on how to make an order form? I need to also include a option where if they are a “non profit organization” then they do not receive the 6.00% sales tax. the 6.00% should be automatically added if they do not click that box
I can call the shopping page my form
I need to sell:
Popsicle $5.99
Dippin Dots $7.99
Bubble Gum $8.99
Push Pops $13.99

I need a quantity box as well
When they press checkout i need it to calculate the total amount due and have that displayed in a text box

Can anyone help me out? This doesn’t need to be that fancy or eye catching.
I appreciate it.

@mrhooApr 21.2008 — Start with the html, then write the server code.

It is always simplest to write the server code before the client code.

Once it is done, the code for javascript will come easily, you will have thought most of it through already.

This is just a real simple form just for me to use and I don't think i will put it on a server so really just html and javascript, but im a little confused on getting it to work on the javascript part. I have looked at a few sources online from forms and its hard to figure it out. I guess ill keep trying though.
