How to execute tag action within form?

I need help with my form.
I have form with code like sample:

[code=html]method=”post” target=”_top” action=”mysubscribe.php”[/code]

How to add also code for action like action=”<?php echo $PHP_SELF; ?>”.

Problem is that need both of action with form. In other words it should be executed file mysubscribtion.php and it should be message on the same page where is form (on my Homepage).
