Submit Form

Hi all!

I would like to create a submit form. Also, a user must register before they submit the form. There will be a link for download but a user must register before they can download the trial version. There is a pull down menu and a comments box. I have a attached an image to give you an idea.

Yes, I am asking how to create this form. Once they click on the submit button, I would like the download process to begin. A pop up window will ask the user either to Run, Save or Cancel. But they MUST submit the form because they can see the download popup window.

Also, can you add another feature of the editable box where a user can enter their friend's email to receive the download page? It's call Send To Friend feature.
