Access from a specified page

Hi gurus,
I am making an ebook and i will upload it to my site but befoer this i want to make sure that a visitor before downloading the book, he or she submits some info like email and answer a certain question.
I thought of developing a form and upload it to “/form.html” and when “download” buton is clicked, visiotr will be directed to a new page containing the download details for example “/download.html”. My worries is that if someone directly access download.html, he will be able to download the book without completing the form.
My question: is there a script that i can insert into /download.html page that would prvent direct access to the page and if so happens it would tell the visitor of the correct page to come from like something “you have to access this page thru /form.html.

I will be very grateful for any help in this matter.

An alternative option is to send the book file by email after receiving the user's email.

Please tell me if there is a scritp I can use. I have no problem in doing the server scripting installaiton. I have done so many of these php and others. I found that if the script has a "readme"file, i can easily follow the istructions.
