multiple form buttons

i am trying to build a web form with two buttons one sends you to one form the second to another. i can’t work out how to do it as i can only have one action atribute and i can’t seem to pass a variable to the action atribute with either php or javascript. it would seem i am approaching the problem from the wrong direction can anyone help?? please many thanks best wishes pat?
