Line 117: -Your "[B]<form>[/B]" lacks a proper closure of "[U][B]</form>[/B][/U]"

Line 132: you are missing a "[B]<tr>[/B]" in this TABLE, [I]and[/I]

Line 119: The form has "<input [B]input[/B] type="submit"...>

(get rid of second "[B]input[/B]")

Any of these might be cause problems (maybe not the problem that you cite). ?
