Hi,
I found this form script and adapted it to my needs.
1.I can not find how to put the following html page in two cols (to the left “Personal data”, next to it to the right “Requested brochures”), the one always is under the other.
2.The “radio” checkboxes (Yes/No) do not work properly. When I click one of them, the other remains checked.
Could you take a look and correct the script?
