creating a question paper in jsrp

i need to create a question paper , which contain fill in the blanks question. Assuming that each question will have single blank to be filled up by the student. so we need a textbox in each question for the student to enter the anz.the a clear button to clear all entered anz. then a correct button to show the correct anz in the blankspace. at the end the grade button to show the total grade depending upon the correct ans, we can assume each correct anz will hav 1mark.
if any can help i will be thankful, i have to submit this on monday.
