Title: Including fan web form on myspace profile

How do I create a web form for a myspace profile similar to the one shown here in the general info located on the right side of this myspace profile:

I would like to capture emails for a music client’s myspace profile from their fans who decide to join the fan club. What is the HTML and procedure for this? Thanks.
