Link To Us Box

Hello everyone. I’ve been searching Google with no answers on how to create a “Link To Us” box. I want something like this:

[CODE]http://javascript.internet.com/link-us.html[/CODE]

I want it to display a button image for my site.

And have the html code to my image button inside the box.

How do I make one, or is there some type of script that does this? I want to add something like this to the footer of my forum board. Any help would be appreciated.
