form and database

I may need a user custom account/login program written! I came across phpautomembersarea at [url]http://www.dwalker.co.uk/phpautomembersarea/[/url]
The program is very simple to install and use, but I am having trouble with a few simple things.

The form and login work perfect for me, but I need to have a few things changed on the form and make sure they are saved in mysql.

The site [url]www.prestigeperksus.com[/url] is where the program is installed. The idea is people will either recieve or purchase a discount card and have to come to our site to activate it. When activating the card people will fill out a form for a user name and password (as seen on the site – go to login). After they have registered they will recieve their password via email and can return to our site to login and get to the protected page which will tell them where they can use their card.

As you can see on the site this works, but whenever I try to change anything on the form (even simple txt) the form returns to the sign in page and doesnt send anything to the database.

If you look at the form I would like to change
“county” to “state”
remove “additional comments”
remove “heard_about_us_from” (remove from tables and add.php)
and add “activation_code” (add to tables and add.php)

I hope I dont sound too confusing. I am just at my limit for now with this and its just a headach and frustrating not knowing what to do. I am surprised that I havent seen many choices in this type or program.

you could register for free and see how it works. Also I like how this program saves user information in mysql for future download – this is very important to us and would need to be available in another program incase we have to change to something else.
