Curiosoty on making a webform….

Hi, first post here and would like some feedback on an issue I am having….

We are a small IT services business and are trying to figure out the best ways for a our on-the-field technicians to report their time and tasks in a fast manner. Here is what we have came up with and the question is: Does anyone know an easy way to produce this webform and an easy way to organize it?

Basically, when the tech is finished we want to be able to go to our site (we host a an ftp site) and fill out the form on any web browser. The form would include: time, date, technician, job tasks, parts used, extra notes, and anything else we can think of. When the form is filled out, we want the tech to be able to save it in a folder and can be pulled up by the billing person to begin billing the client. Any ideas? Something as easy and M$ Frontpage? Thanks for any help anyone can provide!
