Printing Part of Page

I have a page with a short form that needs to be printed so people can mail it in.

I found some code on the Internet and when I first tried it, it worked. Then I made additional changes to the page and when I uploaded it, it didn’t work. For a while when I clicked on “Print this Form” it just took me back to the top of the page. Now I get the error message “Could not find printReady function.”

@IowajonesauthorAug 09.2006 — The last time I loaded the code I forgot to change the designated <div> from "item1" that was the <div> in the original code to "form" which is the <div> name on my page.

Do I feel dumb. Thank you very much for seeing what I couldn't see.
