Using only HTML, CSS, and JavaScript, create an html webpage with a form like the one shown below with the following requirements. You may write the code at the end of this worksheet or attach it in a separate file.
1. When an item is selected from the ‘Item’ pull down, it sets the ‘Quantity’ to 1 and adjusts the ‘Item Total’ and ‘Purchase Total’.2. When the ‘Quantity’ is adjusted for an item, it updates the ‘Item Total’ and ‘Purchase Total’.3. Using JavaScript and whatever HTML additions you need, provide a mechanism for the user to add or remove at least 3 additional items to their order.4. When the form is submitted, check the email address and phone number fields for correctly formatted values. (Assume phone number should be the format XXX-XXX-XXXX)5. Using whatever HTML, CSS, or JavaScript you want, make the page as professional looking and user friendly as possible.
