email friend with php not working

Howdy~
I have no idea on what is wrong with the attached page, but in the your name and email it, is to remember the info the user had inputed. And not sure why I am seeing part of the php on the top of the page. I am not sure if I need to save the attached as a php or can I save it as a html so I can have a link to it? Can someone help me out?

@aussie_girlJul 05.2006 — You save it has .php and you can still put a link to it, the same as you would any other file..
