Forms – Checkboxes and Email Confirmation

I created a simple form and I have everything working but two things. I need to be able to send a Thank You email to the user based on their email address that they enter and when I receive their information they entered in an email, the checkboxes are not showing anything. The checkbox name shows up, for example, [COLOR=Gray]other =, [/COLOR] but there is no value next to it.

I have been trying for days to figure it out and I am hoping someone has an answer.

Basically the form is filled out, they hit submit and I get an email with their information. That works fine.

All I need is the Thank You email to automatically be sent and for the checkboxes to work in the email I receive.
