How do I send 3 values to an action on a form

Hi All,

newbie here, trying to teach myself how to use forms. I’m trying to code a form that has to send three values to the calling script. Attached is the long version, fraught with problems as it allows the user to select the wrong combination of matching values: How can I make the form have just one radio button for each set of three values?

Here's a tip when you want to post code here, when posting code, put it inbetween HTML/CODE/PHP tags, so those who are reading your post can easily get the code. If your having trouble finding the buttons to give you the tags, look right above, there should be "# <>" etc. For this, you could want to click on "<>" and paste your code into the popup dialog! =)
